Evidence helps them to see what life is really like for your child, and for you as a parent or carer. Good evidence makes your application stronger and fairer, because it’s based on real examples and facts, not just opinions.

You can use lots of different types of evidence, depending on what you have. For example:
Letters or reports from school, nursery, health visitors, GPs, paediatricians, therapists or social workers.
GIRFEC Child Plans or other planning documents showing your child’s needs and supports.
Emails or meeting notes that show what has been discussed or agreed about your child’s support.
Videos showing how your child manages daily life, communication, or safety (for example, how they move near traffic or cope with busy places).
Statements from family or friends who know your child well and can describe what they see day to day.
A statement from your child, if they are able and want to share their own views or experiences.
Choose evidence that helps to prove the criteria for the thing you’re applying for.
Try to show a pattern, not just a one-off situation.
If possible, date your evidence, so it’s clear how current it is.
Keep copies of everything you send, as it might be useful for future applications or reviews.
If you don’t have professional reports yet, your own detailed account still counts as evidence. You are the expert on your child.
