If there’s an application form to complete, it usually means there are certain criteria you need to meet. For more information about how to consider criteria when doing an application form, click here.
When you fill in an application form, you might be asked to provide evidence or supporting information.
This simply means showing proof of what you’ve said in your form. Evidence helps the person reading your application to understand your child’s needs, and to see clearly that they meet the criteria for the support you’re applying for.
